2011 Guide to Advocacy - Posted 2-9-11
The 2011 Guide to Advocacy is now available.  This guide contains information about meeting legislators, coalition building, making letters count, understanding legislation and more.  This is a must read guide with the unique challenge of so many new faces in the legislative area this year:  seventy-eight (78) incoming freshman members in the Missouri House, twelve (12) new members in the Missouri Senate, three (3) new members in the United States House and a new United States Senator.

Hospice in the LTC Setting - A How to Guide to Improve Communication and Quality of Life of the LTC Individual Receiving Hospice Care
The Coordinated Task Plan is a recommended process to communicate timely and accurate information when individuals in a long term care community  are receiving hospice.

Click here for The Coordinated Task Plan training booklet which presents an overview of the coordination process developed by LeadingAge Missouri in collaboration with the Missouri Hospice and Palliative Care Association and the Missouri Department of Health and Senior Services.  This collaboration resulted in the determination of necessary information to be communicated and the development of processes and tools to improve communication between hospice and long-term care providers.  The Coordinated Task Plan is available here to be imprinted with the hospice name and logo, if desired.

The American Bas Association Commission on Law and Aging for the National Hospice and Palliative Care Organization has published a "Legal Guide for the Seriously Ill:  Seven Key Steps to Get Your Affairs in Order."  Click here to access to guide.

Continuity of Care Transfer Toolkit
The Continuity of Care Transfer Project is a recommended process to communicate timely, accurate and complete information when individuals are transferred to and from hospital emergency departments nursing units and long-term care communities.  The project's aim is to improve the quality, safety and continuity of care between these care settings.

Resources to Assist Family Caregivers
The Centers for Medicare and Medicaid Services (CMS) has resources to assist family caregivers caring for Medicare beneficiaries.

Click here to access a checklist CMS developed to help individuals and their family caregivers prepare to leave a hospital, nursing home or healthcare setting.  The discharge planing document includes a series of 16 questions for individuals and caregivers to ask themselves and to discuss with hospital personnel to ensure they have a clear understanding of their needs once they return home.
