Hospice in the LTC Setting

A how to guide to improve communication and quality of life of the LTC resident receiving hospice care.

The Coordinated Task Plan is a recommended process to communicate timely and accurate information when long term care residents are receiving hospice care. The project's aim is to improve communication and quality of life in the LTC hospice resident.

The Coordinated Task Plan training booklet presents an overview of the coordination process developed by the Missouri Association of Homes for the Aging in collaboration with the MO Hospice and Palliative Care Association and the MO Department of Health and Senior Services. This collaboration resulted in the determination of necessary information to be communicated and the development of processes and tools to improve communication between hospice and LTC providers. The Coordinated Task Plan form is available to be imprinted with the hospice name and logo, if desired.
